DUTIES AND RESPONSIBILITIES:

  • Practices within the expressed limits of established policies and procedures, securing approval from the Head of Pharmacy for any deviations from the policies and procedures.
  • Reviews chemotherapy and supportive care prescriptions to ensure appropriateness, accuracy, and safety according to oncology protocols and patient-specific parameters.
  • Verifies cytotoxic medication orders in EMR and liaises with oncologists to clarify or amend treatment regimens when required.
  • Ensures compliance with institutional chemotherapy protocols, MOH oncology guidelines, and international standards.
  • Oversees aseptic preparation and reconstitution of cytotoxic and biological agents within the CDR cleanroom in accordance with USP, and Malaysian GMP guidelines.
  • Ensures safe handling, labeling, transport, and disposal of cytotoxic agents according to infection control, occupational health, and DOE standards.
  • Performs batch record verification, daily temperature/humidity checks, and HEPA performance review documentation.
  • Coordinates with Biomedical, Engineering, and QA teams on maintenance and environmental monitoring (airborne particles, microbial sampling, and pressure differentials).
  • Provides comprehensive counselling to oncology patients on chemotherapy and supportive medications, including potential side effects, symptom management, and adherence strategies.
  • Coordinates with physicians, nurses, and dietitians to manage chemotherapy-induced adverse effects.
  • Participates in multidisciplinary tumour board discussions when required.
  • Implements and audits safe handling procedures of hazardous drugs and personal protective equipment (PPE) usage.
  • Reports, investigates, and documents any chemotherapy-related medication errors or near misses as part of patient safety improvement initiatives.
  • Assists in developing and reviewing oncology-related SOPs, treatment protocols, and cytotoxic waste management procedures.
  • Conducts in-house training for pharmacists, pharmacy assistants, nurses, and medical officers on cytotoxic preparation, spill management, and safe handling.
  • Manages procurement, storage, and stock rotation of oncology medications, ensuring compliance with NPRA requirements.
  • Tracks chemotherapy drug utilization, expiry monitoring, and cost optimization through dose-rounding or vial-sharing initiatives.
  • Maintains and updates cytotoxic and supportive medication formulary.
  • Ensures records are compliant with NPRA, CKAPS, and MOH requirements for hazardous drugs and controlled substances.
  • Participates in internal audits and inspections (MOH, NPRA, BOMBA, DOE) related to cleanroom operations.
  • Participates in Pharmacy Department meetings, Pharmacy & Therapeutic Clinical Meeting, other department or hospital committee meetings and quality improvement activities.
  • Undertakes other relevant tasks as may be directed by the Head of Pharmacy from time to time.

EMPLOYEE SPECIFICATIONS AND QUALIFICATIONS

  • Bachelor Degree in Pharmacy (or above)
  • Registered with the Malaysian Board of Pharmacy
  • Possess Poison License A and Annual Retention Certificate
  • Minimum 2–3 years of oncology pharmacy experience
  • Completion of Oncology Pharmacy Training or equivalent certification
  • Strong knowledge of cytotoxic handling, aseptic techniques, and chemotherapy protocols.
  • Able to be on-call (after office hour)
  • Excellent communication and teamwork skills
  • Able to work independently & accountable.
